1. Schedule Appointment

Schedule an appointment with one of our registered physicians and obtain a copy of the Patient Recommendation form from the physician upon completion of your appointment. You will provide this document to the Oklahoma Medical Marijuana Authority when you complete their online registration process.

3. Register for a Patient Card

  • Applications and payment will be collected through an online system. You will need a valid email address to login to this system. This email address will be used to send notices once you create an account.

  • Standard application fee is $100.
