How to Get a Medical Marijuana Card in MI
STEP 1: BOOK AN APPOINTMENT
Schedule an appointment to see a doctor through Green Cross Partners at a time that is most convenient for you. Consult with the doctor for 10-15 mins to evaluate your ailments, and ask any questions you may have about medical marijuana treatment.
STEP 2: CONSULT WITH AN MMJ DOCTOR
After the doctor approves you, you will receive a "Physician Certification Form" for the state of Michigan. Once received, you can register and send your application to the Department of Health for your medical marijuana card. The fee sent to the state is $60.
STEP 3: GET APPROVED
Once you mail send off your completed application (Patient App, Physician Certification, and $60 Payment) the state will send you a new medical marijuana card in the mail in 2-3 weeks. Once your card arrives, you can then begin buying cannabis legally from any dispensary in Michigan.
In Michigan, patients will need to re-certify their license annually by seeing a licensed physician again. Green Cross Partners will get in touch with you when your certification is approaching its expiration to help you setup a renewal consultation.

A signed Physician’s Statement from a physician (provided from a Green Cross Partners doctor), currently licensed to practice medicine in Michigan
Valid proof of residency in Michigan.

Mail your application and Physician Statement to the Michigan Department of Community Health (MDCH) along with the following documents:
Copy of your Photo ID
A check or money order in the amount of the application made payable to "State of Michigan—MMMP"
